Insurance and Fees
We accept most major insurance companies, including Blue Cross Blue Shield, Aetna, United Healthcare, Cigna, Unicare, Humana, North Suburban Associated Physicians, NorthShore University Health System, Medicare, and many others. Please call our office to check if we accept your insurance.
Charges for office visits, consultations, and procedures are determined by the time spent and complexity of the problem. Please feel free to ask what your treatment will cost during your visit.
Medicare
Dr. Goldin is a participating physicians in Medicare and accept assignment on all Medicare claims. We will submit your claim to Medicare for you. Medicare patients are responsible for 20 percent of the Medicare-approved amount plus any deductible. Please be sure we have your secondary insurance information so we can help you file your claim.
PPOs
Dr. Goldin participates in many PPOs. Our office will submit your claim for you, but you will be responsible for any unmet deductibles. You may also be responsible for co-insurance if required by your health plan. Any co-payment due is expected at the time of service.
HMOs
Dr. Goldin participates in many HMOs. Our office will submit your claim for you. If your insurance company requires a primary care referral form, you must bring it with you or bring your referral number. If you do not have this form or the referral number, we cannot submit your claim and we will expect payment at the time of service. Any co-payment due is expected at the time of service.
Cosmetic and Procedure Fees
Insurance companies consider many procedures cosmetic. Dr. Goldin will explain the cost of the procedure beforehand. To save billing costs, we appreciate your payment at the time of service.
Administrative Fees
Due to the time and labor involved, we charge $25 per letter written for insurance, prior-authorization, or work-related issues.
Statements and Payments
We prefer not to send statements because the cost of paperwork and postage affects our fees. Errors in billing can occur, so please feel free to ask our office administrator about any questions you have about your bill. We appreciate your payment by check, cash or credit card at the time of your visit. We accept both MasterCard and Visa. If you need to work out a payment plan for your bill, please let us know so that we can avoid any misunderstandings.
