Making an Appointment with Dr. Goldin
To make an appointment, please call 847-677-2080 during general business hours. While on the phone, we may ask about the nature of your problem in order to determine the appropriate amount of time to serve you best. Our schedule is often very tight, but we do our best to accommodate you in a timely manner. If you prefer not to wait for an appointment, you may call the office each morning for any cancellations. We will call you two days prior to your appointment to confirm the time. To expedite your wait time at your first appointment, we recommend that you complete the “New Patient Form” below and bring it with you to your appointment.

Urgent problems and emergencies
If you have an urgent problem, we will do our best to give you the first available appointment. You should tell the receptionist that your problem is urgent and you will be transferred to a nurse who will help you get the care you need.
Cancellation of Appointments
If you are unable to keep your appointment or are going to be late, please call the office as soon as possible. This courtesy allows us to be of service to other patients. We request at least 24-hours notice for any appointment cancellation. Please see our “Policies” page for more information on cancelling appointments.
Reaching Us After Office Hours
We have a 24-hour answering service for urgent messages and emergencies. Please call the office number (847-677-2080) and when prompted, press 4. You will be connected to the answering service. You may also call the answering service directly at 847-589-4800.
Pharmacy Refills
When you want a refill on a medication that we prescribed, please have your pharmacist fax the request to us (847-677-2098). If the pharmacy wants to call us, please make sure your pharmacist calls during regular office hours. We do not refill prescriptions after hours.
Medical and General Questions
Please call us during regular office hours for non-emergency assistance. One of our nurses or other knowledgeable staff will answer your questions as thoroughly as possible. They will refer you to Dr. Goldin when appropriate.
